The Treatment and Survivorship Work Group workgroup has:

• Developed two Cancer Treatment Summary templates that can be used when survivors complete radiation or medical treatment. These can be edited to meet your needs, but we ask that you credit the NH Comprehensive Cancer Collaboration on the document.
         Medical Oncology
         Radiation Oncology
• Assisted in the development of a brochure for cancer survivors called, Cancer Survivorship: Your Plan of Care.
• Participated in the formulation and evaluation of a web-based survey tool to identify the unmet needs of cancer survivors in NH, and supported a successful grant proposal to fund this project.
• Identified New Hampshire facilities that provide cancer treatment and conduct clinical trials.
